Gratz College will no longer be responsible for ACT48 letters that are returned to us due to an insufficient address. You must update your address with the Department of Education before submitting a request form to the Records' Office.

ACT48 Request Requirements

When ordering by mail, fax, or in person, you must include the following information in your request or your request will be rejected:

• Full Name (First and Last Name)

• Any previous name (as it appears on your student record)

• Social Security Number

• PPID (if SSN starts with zero)

• Home Address

• Phone number (where you can be contacted if we encounter any problems processing your order)

• Email Address (if available)

• Name of Class

• Dates Attended (Start and End dates: All inclusive dates must show the Month, Day and Year)

• Signature and Date
