Employee Accommodation Request Documentation Guidelines
If an employee with a disability wishes to receive an accommodation, it is the responsibility of the individual to inform the Manager of Business Operations, Thomas Cipriano, at tcipriano@gratz.edu and request an accommodation by submitting the Employee Request for Accommodation Form.
The Manager of Business Operations will review and consider all pieces of documentation submitted for employment accommodation requests. Documentation will assist the Manager of Business Operations in understanding how the disability impacts the employee in a work environment and the current impact of the disability as it relates to the accommodations requested. Reviewing documentation is a collaborative process and is handled on a case-to-case basis.
Documentation must be considered current, provide proof of diagnosis, severity, and/or prior accommodations and provided by an accepted healthcare professional. In general, Gratz College follows the guidance from the Association on Higher Education and Disability (AHEAD) on documentation review and accommodation approval.
Employees/applicants should submit the following:
● Employee Request for Accommodation Form
o Instructions provided with Form
● Letter from a healthcare professional, on letterhead, which confirms a diagnosis attached to the Employee Request for Accommodation Form. Note - the provider may not be a family member.
● Authorization for Employee Release of Medical Information
Temporary/Provisional Accommodations
Employees with temporary injuries, such as a broken leg/arm, concussion, or surgery recovery can register with and receive services through the Manager of Business Operations on a temporary basis.
